A Guide to the PLAB test

The Professional and Linguistic Assessments Board test—known as the PLAB test—helps us ensure that doctors who qualify abroad have the right knowledge and skills to practise medicine in the UK.

Do you need to take the PLAB test?

If you graduated from a medical school outside of the UK, European Economic Area (EEA) or Switzerland, you’ll probably need to take the PLAB test. The PLAB test will check that you know and can do the same as a doctor starting the second year of their Foundation Programme training in the UK.

What is the UKMLA AKT (PLAB 1) Exam?

UKMLA AKT (PLAB 1) is the first part of the Professional and Linguistic Assessments Board (PLAB) test, designed for international medical graduates who wish to practice medicine in the UK. It evaluates whether their medical knowledge and skills meet the standards required to work as a doctor in the UK. While UKMLA AKT (PLAB 1) is a written multiple-choice exam, UKMLA CPSA (PLAB 2) involves a practical assessment (OSCE). There are two parts to the test. Part 1 is a written multiple choice exam, with 180 single best answer questions. Part 2 is a practical objective structured clinical exam, known as an OSCE. You’ll need to pass both parts before you can apply for registration with a licence to practise medicine in the UK. 

UKMLA AKT (PLAB 1) is offered in multiple locations globally, including both the UK and several international cities. To check where the exam is available, visit the General Medical Council (GMC) website. 

The exam consists of 180 multiple-choice questions, each requiring you to choose the single best answer from five options. Although more than one answer might seem correct, your task is to select the most appropriate one. The test duration is 180 minutes.

Is UKMLA AKT (PLAB 1) Difficult?

With proper preparation, PLAB 1 is very achievable. The exam does not focus on memorizing lists or rare details—for example, it won’t test how many causes of right-sided heart failure you know. Instead, it assesses how well you can apply your medical knowledge in clinical scenarios.

It assumes that you already possess the foundational medical knowledge (e.g., from an MBBS degree) and now aims to assess your ability to apply that knowledge in a UK healthcare setting. Therefore, it is essential to base your preparation on UK best practices and guidelines, which may differ from protocols in other countries.

The exam typically focuses on two main types of cases:

1. Acute presentations – commonly seen in emergency or urgent care settings.

2. Chronic conditions – usually managed in primary care or general practice.

Questions may involve identifying the most likely diagnosis from a set of symptoms, determining the most appropriate investigations or treatments, or interpreting clinical data such as blood test results or imaging reports. You might also encounter basic statistical questions, all designed to assess your clinical reasoning and decision-making abilities in a UK context.

When can you take the exam?

PLAB 1 is held four times a year — in February, May, August, and November. The exam is conducted in the UK and at selected international locations.

Please note:
1. Overseas bookings close six weeks before the exam date

2. UK-based bookings close two weeks before the exam date

3. All exam places are limited and in very high demand

We encourage candidates to plan early. The GMC closely monitors bookings to ensure fair access throughout the year.

Keep in mind: The number of UKMLA AKT (PLAB 1) exam seats is not connected to the number of available jobs in the UK healthcare system.

Where can you take the exam?

The UK exams currently take place in: Belfast, Birmingham, Cardiff, Edinburgh, London, Manchester, Oxford and Sheffield.

The exam can also be taken in two EU locations: Dublin and Madrid. If you’ve secured a booking at a UK or EU location, you’ll receive confirmation of your venue address within 7 days of your exam.

This information will be sent to you by the GMC approved supplier VICTVS, who are responsible for delivering and invigilating UK and EU based PLAB 1 exams on behalf of the GMC. If your booking is for an exam taking place outside of the UK and EU, you’ll receive venue address details directly from the British Council.

Once you have booked online, please only contact VICTVS or the relevant British Council office if you have not received details of the venue at least one week before the exam. Contact details for all exam locations are listed below.

What happens after you’ve passed UKMLA AKT (PLAB 1) and UKMLA CPSA (PLAB 2)?

Once you’ve passed both parts, you can apply for registration with a licence to practise. Your application must be approved within two years of passing part 2 of the test. Once we approve your application and you gain registration you can work as a doctor in the UK.

UKMLA CPSA (PLAB 2)

UKMLA CPSA (PLAB 2) is an objective structured clinical exam. It’s made up of 16 scenarios, each lasting eight minutes and aims to reflect real life settings including a mock consultation or an acute ward. Part 2 tests run throughout the year at our clinical assessment centre.

Overview of UKMLA CPSA (PLAB 2) Exam
  1. It is an Objective Structured Clinical Examination (OSCE) aligning with the UK Medical Licensing Assessment standards.
  2. 16 clinical stations in total
  3. 8 minutes spent at each station
  4. 90-second interval between stations to reset and prepare
  5. Two designated rest stations – time to pause and regroup
  6. Exam duration is approximately 3 hours from start to finish
Exam Format & Assessment Domains

Each station assesses candidates across three domains:

  1. Data-gathering, technical & assessment skills – history-taking, physical exams (e.g., CVS, respiratory, abdominal, mental state), practical tasks, interpreting investigations.
  2. Clinical management skills – making diagnoses, explaining care and management plans.
  3. Interpersonal skills – establishing rapport, informed consent, communication clarity, professionalism.

These domains reflect real-life clinical and professional expectations for an F2-level doctor in the UK

What Happens on the Day
  1. A mix of simulated patients (actors), manikins, and equipment may be used in different scenarios.
  2. You’ll be observed by an examiner (in-person, phone, or via camera) who does not intervene, except in rare cases.
  3. You must not bring books, electronic devices, watches, or write down exam content during the test.
  4. Lockers are provided: bring minimal belongings, switch off phone and store before
    entering the circuit.
UKMLA CPSA (PLAB 2) test dates

PLAB 2 test dates throughout the year. You can view available dates in our PLAB booking service via GMC Online. You can only do this once you have your PLAB 1 results.

When can you apply for an additional attempt?

To be eligible for an additional attempt for either PLAB 1 or 2 (after your fourth failed attempt), there must be at least 12 months between your fourth failed attempt and requesting an additional attempt.

You should undertake further learning to improve your medical knowledge and clinical skills. You must have completed either 12 months’ clinical practice or a post graduate qualification. If you have already had more than four failed attempts at either part of PLAB 1 or 2, you are no longer eligible to take the test or apply for an additional attempt.

If you are granted an additional attempt at PLAB 1, you may need to submit valid English language evidence for verification before you are able to book. If you are granted an additional attempt at PLAB 2, you may need to sit PLAB 1 again if your PLAB 1 validity has expired. This may also mean you have to submit further English language evidence in line with our PLAB 1 eligibility criteria.

How will you be tested ?

During the exam you will be marked against three areas or ‘domains’ for each scenario.

Domain

Data gathering, technical and assessment skills

History taking, physical examination, practical procedures, investigations leading to a diagnosis

Clinical management skills

Formulating a diagnosis, explaining something to the patient, formulating a management plan

Interpersonal skills

How you approach the station: whether you establish a rapport with the patient, how you use open and closed questioning, involving the patient and demonstrating your professionalism and understanding of ethical principles

When marking against the domains, the examiner will assess your competency across a number of skill areas.

Clinical Examination

Most of the stations will involve a simulated patient. You should interact with the actor as you would a real patient, including requesting to perform any relevant clinical examinations.

You should begin each station by confirming your name and GMC number. This is important as the examiner may not always be present in the room and may be watching remotely.

In some rooms you will find telephone consultation stations; the call will be connected for you before you enter the room. It is important you do not touch the phone or monitor as this will disconnect your call.

In some stations, you will be assessed on your ability to conduct a physical examination of the simulated patient, or using a model or manikin.

We expect you to be able to carry out any basic physical examinations, including but not limited to examination of the abdomen, breast, chest, hand, heart, and joints. You must be able to perform a rectal or bimanual vaginal examination. You must also be able to use the appropriate equipment in carrying out an examination of the ear, eye or nervous system.   

You may be required to perform an examination or other procedure on a high-fidelity simulator which can be programmed to show normal and abnormal clinical signs. A role player may speak from outside the room using a microphone connected to the simulator. You can find more information and a video on the manufacturer’s website. Examination of someone’s mental state is a form of clinical examination for the purpose of the PLAB 2 exam.

The examiner will assess you on your ability to perform common practical procedures.

The practical skills may include:

  • checking blood pressure

  • performing venepuncture

  • inserting a cannula into a peripheral vein

  • calculating drug dosage

  • giving intravenous injections

  • mixing and injecting drugs into an intravenous bag

  • giving intramuscular and subcutaneous injections

  • basic cardio-pulmonary resuscitation (adult and child)

  • suturing

  • interpreting an electrocardiogram (ECG), X-rays or results of other investigations

  • interpreting basic respiratory function tests

  • performing urinary catheterisation

  • taking a cervical smear

  • Safe disposal of sharps.

We test these skills by observing the interaction between you and another person, usually a simulated patient or occasionally the examiner. These interactions can take place in person, in writing (eg, a prescription), or via telephone call. The examiner will assess your approach to the patient throughout the examination, even if they are not present in the room.

This may include:

  • explaining diagnosis, investigation and treatment

  • involving the patient in the decision-making

  • communicating with relatives

  • communicating with health care professionals

  • breaking bad news

  • providing clear, legible written instructions where appropriate (eg. prescribing)

  • seeking informed consent/clarification for an invasive procedure or obtaining consent for a post-mortem

  • dealing with anxious patients or relatives

  • giving instructions on discharge from hospital

  • giving advice on lifestyle, health promotion or risk factors.

As we may use an anatomical model in some stations it’s important to remember the following: 

  • Tell the examiner what you are doing and why only if the instructions tell you to.

  • Don’t perform any actions on an anatomical model that would be unsafe or painful for a real person.
